- Resource One Health Connection is an equity-focused initiative connecting low-income rural Missouri residents to affordable, quality care. Operated by the Family Resource Center of Cass County, the program offers one-on-one Medicaid navigation, care coordination, and medical referrals. On-site health bots conduct screenings and alert telehealth nurses in real time for high-risk clients. Care Coordinators distribute blood pressure monitors, glucose kits, and first aid supplies on a case-by-case basis to promote chronic disease management. By addressing both clinical and social determinants of health, the program improves outcomes and builds sustainable, community-based healthcare access.
- Emergency Assistance provides short-term support to individuals and families facing financial hardship, often related to housing, utilities, or other essential needs.
- Ready 4 Change helping individuals transition back into society after being incarcerated, providing support to reduce recidivism and promote successful reintegration. We offer a range of services, including referrals to local resources, employment support, and access to healthcare, and addressing the various challenges that formerly incarcerated individuals face.
